Express Mail Application Form

This agreement between JTC’s Mail Service, herein referred to as “agent”, and the subscribing applicant, referred to as “addressee”, made in Anguilla, B.W.I., agree to the following terms:

• Agent is the subscriber to a certain mail service addressed at 6800 SW 40^th Street, Miami, Florida. 33155, and the said address will be subject to the agent’s clientele referred to as “Addressee”.
• The minimum rental period shall be no less than one year. All subscription fees are payable in advance.
• Subscription fees applicable to renewals of the mail service are due and payable yearly on the 1^st of the month of the original subscription date.
• Agent shall refuse to accept on behalf of Addressee “postage due” mail and or COD items unless special arrangements are made in effect beforehand.
• Addressee agrees that mail and or parcels are said delivered upon their placement or placements of notice of availability in assigned mailbox or mail bin and relieves agent of any further performance responsibility.
• If you are attempting to avoid creditors or the Law, do not rent a mailbox here. WE WILL NOT SHIELD YOU.  JTC’s Mail Service will cooperate with local and federal agencies both in Anguilla and the United States of America.  JTC’s Mail Service will release information about you to such agencies and all postal inspectors upon request.  JTC’s Mail Service also reserves the right to immediately terminate service upon suspicion of fraud or suspected illegal activities.
• Use of your mail service account by unauthorized users will be subject to an unauthorized user fee based on the applicable rate in effect at the time.
• Upon termination of this agreement, agent shall hold any mail of the Addressee for a period of not more than thirty (30) days after agreement termination.  After 30 days, agent will dispose of said mail. ADDRESSEE UNDERSTANDS THAT THE US POSTAL SERVICE WILL NOT ACCEPT CHANGE OF ADDRESS UPON TERMINATION OF THIS AGREEMENT. IT IS THE ADDRESSEE’S REPONSIBILITY TO NOTIFY CORRESPONDENTS OF ANY CHANGE OF ADDRESS.
• PRICES ARE QUOTED IN UNITED STATES DOLLARS (USD) AND ARE SUBJECT TO CHANGE WITHOUT NOTICE.
